What is County Managed Care?
County Managed Care refers to a system that provides coordinated delivery of services and supports that are necessary and appropriate, delivered in the least restrictive setting and the least intrusive manner.  Managed Care seeks to balance three factors:
1.  Achieving high quality outcomes for participants
2.  Coordinating access
3.  Containing Costs

Who is eligible for County Managed Care?

  • Persons living in Hancock, Winnebago, or Worth Counties or persons having legal settlement in Hancock, Winnebago, or Worth Counties
  • Persons needing Services and Supports paid from the MH/DD Mental Health Fund
  • Persons requesting assistance with mental health needs
  • Persons receiving mental health treatment through the court system.

What is Central Point of Coordination?

  • The administrative entity designated by the County Boards of Supervisors to act as the single point into the service system, as required in section 331.429, Iowa Code.
  • The single entry point for all consumers needing services and supports paid for from the Senate File 69 MH/DD Mental Health Fund.
  • The service and cost tracking for all Services and Supports to be paid out of MH/DD Mental Health Fund.
  • The entity that pays only for preauthorized services and supports.  Payment will be made only for preauthorized Services and Supports.
  • The educational component responsible to provide information to consumers, families, providers, and the court system to make the delivery system work smoothly.

What type of Services and Supports are paid for from the MH/DD Special Services Fund?

  • Mental Health Evaluations through Court Commitments
  • Mental Health Counseling through Mental Health Center of North Iowa
  • Attorney Fees through Court Commitments
  • Sheriff's Transportation through Court Commitment
  • Mental Health Advocate
  • Home Care Aide
  • Supported Employment
  • Supervised Apartment
  • Residential Care
  • Residential Care for Mentally Retarded
  • Intermediate Care for Mentally Retarded
  • Work Activity
  • Sheltered Workshop
  • Home and Community Based Waivers
  • Targeted Case Management
  • County Social Worker
  • Respite
  • General Relief

How do I access services and supports?

  • Through the court system for mental health commitments
  • By completing a Central Point of Coordination Application and returning to WHW County Social Services

  • From referral through counselors, providers, family members, consumers to Hancock, Winnebago, and Worth Counties

Will I have to pay for services and supports?

  • Services and Supports could be free to you if you are below the 125% Federal Poverty Levels
  • You may still qualify for Services and Supports if your income is over the Poverty Level; however, you will have to pay for the Services and Supports.

What is considered as income and resources?

Income is defined as all earned and unearned income of applicant, their spouse, and their minor children.

Resource limitation is $2,000.00

  • Persons whose income is below 200% of the Federal Poverty Levels of 2005
